Performs an age specific plan of care for a designated group of patients using the nursing process of assessment, diagnosis, outcome identification, planning, implementation and evaluation of patient care.

Graduate from an accredited School of Nursing, Associate’s Degree in Nursing required, Bachelor’s Degree in Nursing preferred, and a current license to practice as a Registered Professional Nurse in New York State and/or Connecticut.

Assesses the patient’s physiologic health status by conducting patient interviews, explaining policies and procedures, reviewing the patient’s chart, and assessing various health statuses including gastrointestinal, cardiovascular, respiratory, renal, and neurological.

Develops criteria for measurement of patient outcomes, identifies actual/potential patient problems, and establishes individualized patient goals.

Maintains constant vigilance over patients care, exercises professional skills related to the plan of care, reassesses patients as needed, and provides emotional support to patients and their significant others.

Administers medications correctly and safely, identifies medication action, dosage, side effects, and implications for the patient, and educates patients and significant others related to drug and food interactions.

A minimum of 1-2 years of professional nursing experience in the related specialty.

BLS is required, plus specialized certifications as needed.

Provides explanations of the patient’s condition, communicates assessment data, recognizes health teaching opportunities, and evaluates the effectiveness of teaching by feedback from patient/family.
